Advancing Healthcare Transformation Throughout The Southern Tier

Care Compass Network (CCN) is a not-for-profit, community organization formed in 2015 to advance the transformation of health care delivery through the development and convening of strong and resilient partnerships, the coordination of care, and the reduction of health care expenses. CCN convenes and collaborates with over 115 partner organizations, which include hospital systems, community organizations, social care/ human service providers, skilled nursing facilities, behavioral health and substance use disorder programs, local county governments, and higher education systems.

In January 2022, Care Compass Network launched the Open Network to assist organizations and their employees in developing the necessary skills and expertise to create sustainable, high-quality services and programs that better the quality of life for community members, improve health equity, and serve the most vulnerable across our region.

Under the Care Compass Family of Programs umbrella is Care Compass Collaborative (CCC) and Care Compass Supporting IPA (CCS-IPA), LLC. Care Compass Collaborative was established as the foundation that allows the overall Care Compass Family of Programs the ability to secure grant funding and provide certain services, at a cost, to organizations through our Custom Solutions. Care Compass Collaborative is the sponsoring entity for the Care Compass Supporting IPA which has been approved by the NYS Department of Health and will be a focal point in the implementation of new managed care programs impacting Medicaid and Medicare through Value-based Payment (VBP) initiatives and the integration of Social Determinants of Health into clinical care. This new, non-traditional IPA serves as the contracting vehicle for the Performance Network that will strengthen and integrate social care and clinical services through the participation in VBP contracts. Unlike a traditional IPA, which is typically comprised of primary care providers and other clinicians, the social care IPA will primarily represent organizations who address social determinants of health needs as well as smaller clinical organizations that typical don’t have the size and scale to participate in a VBP contract.

The Care Compass organizations are separate and distinct legal entities as will be indicated within this website and upon any engagement with the Care Compass Family of Programs. CCN is the sustaining network exempt under 501(c)(6) of Federal tax code, with CCC as the foundational and charitable organization exempt under 501(c)(3). These entities are not licensed as providers of health care. Care Compass Supporting IPA, is expressly approved to coordinate and integrate services as well as payment in relation to managed care initiatives with providers of care as well as Community-based Organizations (CBOs) involved in social services. “Care Compass” may refer to the umbrella of entities and programs sponsored by or initiated by the Care Compass Family of Programs which are designed to effectively, efficiently, and legally meet the needs of our changing health and social service delivery system.

Our Mission

The mission of Care Compass Network (CCN) is to improve the health and well-being of the community members in the CCN service area by supporting the development of enduring partnerships of clinical and community service providers and empowering those partnerships to flourish in a value-based payment environment.

Our Vision

By 2025, CCN envisions a care delivery environment that is more collaborative and better structured to deliver services that are integrated and coordinated around a person’s needs. CCN will advance the transformation to supportive systems of care as a:
