Care Compass Supporting IPA
Care Compass Supporting IPA (CCS IPA) is a resilient, innovative, and collaborative network of community-based organizations that work collectively to integrate and strengthen clinical and social care interventions and services, thereby minimizing the health and social inequities to promote access, connection, inclusion, and well-being of our community. CCS IPA will organize a regional social care network and will serve as the primary convener of the network. CCS IPA will also provide program administration functions to alleviate the burden on community-based organizations pertaining to network management, contracting, data collection and management, and strengthening workforce capacity.
