In anticipation of the Social Care Network announcement from NYS, Care Compass continues to prepare for the development and implementation of a Social Care Network and further the advancement of integration of health and social care across the region. Additionally, we will share information on the recently selected closed loop referral platform, Unite Us, to be utilized with current and future initiatives that support further transformation.
