Care Compass Collaborative (CCC) overviewed the participation requirements for organizations interested in providing services as part of the Social Care Network. There was also an update regarding CCC’s IT platform, Unite Us, which is being offered to network participants in support of creating shared end-to-end visibility of the Member journey from Health-Related Social Need (HRSN) Screening and Navigation through delivery of HRSN services.
