Under the NYS Health Equity Reform (NYSHER) 1115 Waiver, NYS Department of Health has incorporated a mandatory screening tool that will be utilized by partners in the Social Care Network. Depending on the SCN region, this tool will be incorporated into the SCN’s central platform or the partners own system of record. NYSDOH has selected the AHC Health-Related Social Needs Screening Tool as the screener for the 1115 waiver.
