Care Compass was recently asked to support and facilitate collaboration between area hospitals, provider groups, community organizations in a joint effort to improve the quality and healthcare outcomes for Medicaid members with Behavioral Health and/or Substance Use Disorder conditions. Care Compass’ role will be to facilitate planning meetings, convene community organizations to provide social services to the target population, provide data analytics, and project management to support this new initiative.
This new demonstration project is called the Southern Tier Integrated Collaborative Hub (STICH). This new project is a region-wide initiative that is looking to combine resources, experience, and efforts to support individuals with behavioral health and/or Substance-Use Disorder conditions by expanding access to services in the community setting and deploying targeted interventions to improve health outcomes. The STICH pilot while provide experience to improve readiness for advanced VBP contracting and will help providers and community-based organizations to gain knowledge in supporting a specific target population on a regional level.
The initial STICH pilot project is expected to launch in early September 2023. Care Compass will solicit participation from community-based organizations based on the identified interventions to be deployed. This outreach process is expected to begin in June.
