To achieve CCN’s mission of improving the health and wellbeing of the community, CCN is committed to supporting its partners in reducing health disparities and promoting health equity across the region. The CCN Health Equity Council was formed in early 2022 to develop and advise on implementation of a Health Equity Plan, which will support existing health equity efforts in the region, increase the diversity of representation in CCN’s work, and improve awareness of health disparities and health inequities.
CCN’s Health Equity Council in comprised of nine partner organizations representing expertise in Communities/People of Color, Gender Minorities, Substance Use and Mental health, LGBTQ Community, Rural Population, Low Income Households, Domestic Violence, Housing, Justice Involved Population, and Education.
The Council has prioritized three target populations to focus on. They are; People of Color, Substance Use and Mental Health, and Low-Income Households. The council will be creating three sub-groups for those targeted populations to include workers who engage with the targeted population and people from the community with lived experiences. These subgroups will be the sounding board for ideas to strengthen and improve awareness of health disparities and health inequities.
