In communities across the region, people face challenges that cannot always be solved by medical care alone. Housing instability, food insecurity, lack of transportation, limited income, and other unmet social needs create barriers to achieving better health outcomes and quality of life. Our Social Care Network is working to meet those challenges head-on by bringing healthcare providers and human services/ social care organizations together, so people don’t have to navigate these challenges and systems alone.
At its heart, the Social Care Network program is about connection—connecting people to the right services at the right time.
Nourishment Beyond the Plate
In the CCC Social Care Network, organizations like the American Nutrition Alliance are making healthy eating more accessible, practical, and empowering. As shared by the Women’s Opportunity Center, “the food provided from American Nutrition Alliance looked really healthy. I was jealous when someone made blueberry pancakes from the ingredients provided to them.” Not only do clients receive fresh ingredients delivered directly to their home, they also have access to a user-friendly app that allows them to choose healthy recipes.
The result is more than meals. When someone proudly makes blueberry pancakes from scratch, it reflects a deeper shift. It’s confidence in the kitchen, encouragement to cook, and a renewed relationship with food that supports long-term wellness.
Building a Stable Foundation
For many, the journey begins with housing.
The staff at Village at Ithaca know that even the most motivated clients cannot move forward without financial stability. Poverty is not a lack of effort or knowledge; it is a lack of resources. Through partnership with Care Compass Collaborative, Village at Ithaca can now tell families something life-changing: “your security deposit and first month’s rent are covered.” This really makes all the difference to families and people in need.
That moment of relief creates space to breathe, plan, and believe again. For families and individuals in crisis, those funds are not just rent payments, they are the foundation for stability, dignity, and hope.
This same foundation transformed the life of a recent college graduate. They spent ten months homeless—living in a storage unit or an outdoor hallway—working part-time but were unable to afford a security deposit to secure a stable place to live. Initially they were ineligible for enhanced services through the Social Care Network program, but through the diligent work of the Social Care Navigator at Family Enrichment Network, they worked closely with the individual and their primary care physician to enroll them in the 1115 Waiver program.
That dedication to helping others changed everything.
With Family Enrichment Network’s hands-on support, rent and utilities were paid, and a new apartment secured. Now that same individual holds a full-time job and has a safe, stable home—for themselves and their dog. It is a powerful reminder that when systems work together, we can turn barriers into pathways.
From Survival to Purpose
One client of Opportunities, Alternatives, and Resources (O.A.R.) of Tompkins County has lived in survival mode for over 15 years; struggling with substance use disorder, cycling through incarceration, homelessness, and short-term recovery programs. Over time, they shared with O.A.R. that they had come to believe “that opportunities in education, employment, and stability were not meant for someone like me.”
Everything shifted when care became coordinated.
Through the hard work and dedication of the staff at O.A.R. and their partnership with the Social Care Network, they were able to secure stable housing. That stability created the foundation to focus on recovery and personal growth instead of day-to-day survival. With continued social care support, they have gained employment, purchased their first vehicle, and are working towards an associate’s degree in Chemical Dependency, with long-term goals of pursuing a law degree.
Perhaps most importantly, they began to challenge the belief that they were “…less than. I now recognize that this belief was learned—it was shaped by my environment and experiences—but it was never the truth. The truth is that I am capable of growth. I am capable of learning, working, and building a meaningful life. I am deserving of the same opportunities as anyone else.” Today, they are managing their stress, building healthy relationships, and seeing themselves not through the lens of their past, but through the possibilities of their future.
Their story is not just one of individual resilience, it is a testament to what happens when housing, transportation, employment, and recovery supports are aligned around a person’s full life experience.
One Network, Many Paths Forward
Each of these stories is different. A family securing housing. A graduate finding stability. A person in recovery reclaiming purpose. A household learning to nourish themselves well.
What connects them is the Social Care Network program.
Our network of service providers are working behind the scenes to ensure social care is not fragmented or overwhelming. By aligning healthcare, community organizations, and health-related social needs services, the Social Care Network helps create the conditions where people can move from crisis to stability, and from stability to growth.
Success is not measured only in referrals or services delivered. It is measured in secure housing, first paychecks, shared meals, degrees pursued, and futures reclaimed.
